Bilateral Primary Congenital Glaucoma in a Newborn

Physical examination at birth of a male term newborn revealed bilateral corneal clouding and anisocoria (left > right) (Figure). The newborn had been delivered via vacuum-assisted vaginal delivery to a gravida 2, para 1) mother after an uneventful pregnancy. Birth weight was appropriate for gestational age. Apgar scores were 7, 8, and 9 at 1, 5, and 10 minutes, respectively. No risk factors for infection were identified. The parents were healthy and nonconsanguineous.

A 13-year-old male with papillary thyroid cancer status post total thyroidectomy and bilateral neck dissection 2 days prior presented with acute onset of upward right gaze. The episode lasted for 15 minutes, self-resolved, but occurred again 2 hours later. On neurologic exam, the patient was awake, alert, and able to follow commands but voiced significant discomfort. His pupils were equal, round, and reactive to light. His eyes were fixed upward and to the right, he was unable to track the examiner in any direction during extraocular muscle testing.
